This questionnaire is for any organisation that provides positive activities for young people aged 13-19 or up to the age of 25 for those with learning difficulties or disabilities in South Gloucestershire.

Please refer to the consultation background paper and the list of activities that have been funded by the Positive Activities Subsidy.
